Obviously you are not too concerned about your oral health if you have avoided the dentist for years. HIV positive people are prone to oral health issues. As a matter of fact, immune system decline/failure usually shows up in the mouth first because of the large amount of bacteria in the mouth. I suggest that you pursue finding a HIV patient friendly/educated dentist in oral health management, complete exam annually, cleaning every 6 mos. Low dose Valium can help with fear issues (common issue)

The most this heavy reassurance will facilitate in the terrified dental patient is to allow the hygienist and dentist to take a look. This won't facilities these professionals to undertake any actual treatment. These patients will very likely need sedation or else even a general anaesthetic for even prodding or scraping. In the UK since the late 1970s an anaesthetist has to be present for any procedures performed by sedation. And of course an anaesthetist must be present for general anaesthetic.
