When I ceased taking medications my cd4 count dropped from over 400 to 10. Six years later of taking medications daily the counts are yet to pass the 200 mark. Australia has a reasonable health care system but there's no follow up of PLWHAs to chase up if they are missing appointments.

I'm familiar with people with mental illnesses skipping pills because they "hate taking them", or because it "reminds them that they need them". Clearly daily anti HIV meds need to be taken as much as daily antii mental illness pills. In both cases the daily pill keeps the level of drug in the system constant. In both instances a constant level is needed to be maintained. You'll notice it quickly if you skip anti mental illness pills, but you won't notice it so well if you skip anti HIV meds.
