Salary Range: $40,000-$45,000

 

Job Summary:

The Active Aging Specialist will assist our Active Aging Manager to expand and enhance current programming by providing direct, skilled client services in two priority areas:

  • Create and organize workshops, support groups, and seminars at The Center and in the community for LGBTQ+ Active Agers, including the provision of activities on nights and weekends for working older adults;
  • Provide direct, individualized, non-clinical care-linkage services on-site and in the community for Active Agers in need of culturally competent providers.

 

Responsibilities:

  • Create and organize, market, maintain a budget, and facilitate workshops and seminars for LGBTQ+ Active Agers during evening and weekend hours. Events will feature skilled speakers or moderated panels of community experts on topics including, but not limited to, the Affordable Care Act, Wellness and Nutrition, Social Security, Long-Term Planning, Medicare, Relational Skills, and Legal needs.
  • Provide basic non-clinical care linkage and patient navigation for LGBTQ+ Active Agers to culturally competent healthcare providers; link LGBTQ+ Active Agers to culturally proficient medical, mental health, social services, and groups
  • Help coordinate, organize, and implement activities and services to increase the social support, health information, education, connection, and resource linkage of LGBTQ+ Older Adults
  • Create and organize a series of social/educational events at various long-term care facilities throughout South Florida. Events will target LGBTQ+ residents and their peers and open the dialogue for diverse people to engage with The Pride Center and each other
  • Facilitate communication with Carrfour staff at The Residences at Equality Park to ensure residents gain access to Active Aging care linkage, events, activities, workshops, and seminars at The Pride Center
  • Help Manager organize, market, and facilitate all activities for weekly Coffee and Conversation events and other Active Aging services
  • Assist Manager with the coordination of the annual Active Aging Health Expo
  • Assist Manager coordinate strategic social media and marketing outreach for Active Aging events, groups, and activities. Collaborate with Active Aging Manager and Marketing and Communications Manager to develop appropriate outreach/marketing materials for workshops and seminars
  • Prioritize diversity, equity, inclusion, and accessibility in program planning and implementation
  • Collaborate with Customer Services Coordinator to ensure recruitment, training, supervision, evaluation, and recognition of volunteers necessary for activities, events, and outreach
  • Attend and participate in outreach/education activities as deemed demographically and programmatically appropriate to the LGBTQ+ Active Aging program, including coordination with community agencies and partners
  • Collaborate with the Events Coordinator to ensure proper logistics-planning for workshops, seminars, and events, adhering to the Events Planning Timeline
  • Recruit, schedule, and manage appropriate speakers, presenters, exhibitors, facilitators, educators, health care providers, panel members, and social service representatives for workshops and seminars
  • Organize all necessary logistics for scheduling, food, audio-visual and room set-up for seminars and workshops
  • Provide individual coaching and support services to facilitate navigation of complex healthcare, housing, financial, and support-service systems
  • Work with individual LGBTQ+ Active Agers to assess needs, develop care plans, and coordinate services through advocacy, education, support, empowerment, and coaching
  • Ensure the collection, collation, and organization of appropriate on-line links, hard-copy forms, eligibility requirements, and data related to intake, resource, and referral information from major housing, assisted living, healthcare systems, and social service providers for older adults
  • Help ensure updating, expansion, and provision of Active Aging linkage resources. Organize a centralized resource “library” of relevant links, forms, and paperwork for access to these providers
  • Assist the Manager to address cultural competency and readiness of service providers to work with LGBTQ+ older adults
  • Develop user-friendly flow charts and “decision trees” outlining eligibility requirements and major contacts for appropriate social support, healthcare, and housing systems for use with clients
  • Arrange for termination or smooth transfer from Center services as appropriate for clients
  • Record and track workshop and event participant data and demographic information
  • Maintain, document, and regularly report accurate participant attendance and demographic data to meet any Center and funder requirements
  • Design, distribute, and collect self-report surveys at every workshop or seminar to measure the increase in knowledge and access to service linkage
  • Adhere to strict procedures of confidentiality, release of information, and data security
  • Provide outreach, health education, and support in conjunction with other Pride Center teams and community members, including active leadership in major Center and community events
  • Develop close working relationships with internal team members and external agencies to ensure coordinated, comprehensive client services and to avoid duplication
  • Maintain an organized office space and professional communication standards, including prompt return of emails and phone calls
  • Submit reports, invoices, receipts, and requests to the Active Aging Manager in a timely and organized manner
  • Meet regularly with Active Aging Manager for proper planning, reporting, and program evaluation
  • Attend “All-Staff” meetings and trainings
  • Comply with The Pride Center Code of Conduct and Employee Handbook
  • Other activities as assigned by Active Aging Manager and COO/CEO

 

Minimum Requirements:

  • Bachelor’s degree in Social Work, Senior Services, Adult Education, Healthcare, or related field; OR equivalent years of direct experience in a related field
  • Proven experience in case management, social work, elder/Seniors services, ALFs, education, outreach, and/or healthcare
  • Experience with and knowledge of Active Aging and LGBTQ+ communities and issues
  • Exemplify and embrace The Pride Center’s commitment to diversity, equity, inclusion, and accessibility
  • Experience in recruiting and organizing volunteers
  • Ability to train volunteers to accomplish duties and special projects
  • Technology skills with social networking sites, social media, and outreach to LGBTQ+ communities
  • Experience in using social media (including Facebook, Instagram, or Twitter) as an outreach tool
  • Computer skills using Microsoft software applications (e.g., Word, Excel, Outlook, PowerPoint), IOS, and other operating systems, as required. Experience with Apple preferred.
