The Care Navigator is responsible for providing care navigation services to clients enrolled in Housing Works Youth and Prevention Services West 37th Street site. The Care Navigator builds strong relationships with Black and Latinx MSM and transgender women or men that reside in New York City. Conduct targeted case finding in areas where the focused population congregate. The Care Navigator conducts GPRA Intake and Follow Up assessments, develops individual care plans, facilitates T-SISTA and Seeking Safety Groups, and manages overall care coordination for all enrolled clients.

• Conduct SAMHSA GPRA Intake and Follow up assessment and case management activities, including linkage to care and integrated case conferences; coordinates with other service providers regarding linkage and referrals
• Participate in community events, such as health fairs, LGBT events, testing events, etc. to engage new at-risk clients for prevention services and treatment services
• Refer clients to Housing Works Behavioral Health Services, Medical Services, Housing Services
• Strong focus on Educating and linking HIV Negative clients to Biomedical Prevention Services
• Responsible for connecting all newly identified HIV positive clients to HIV Care
• Track client attendance at medical and behavioral health appointments and initiate outreach to clients with missed appointments, as necessary
• Participate in reoccurring clinical meetings with Care Manager and Program Coordinator to discuss retention issues, and internal CQI projects
• Maintain documentation of all client encounters and complete reporting requirements according to organization standards
• Accompany clients to medical appointments, HRA appointments, when required
• Facilitates Evidence Based Intervention group
• Manage organization and documentation of engagement/outreach efforts
• Participate in supervisory sessions with the Program Coordinator
• Perform other duties as assigned
