Summary:

The Care Support Associate will assist the Emotional Health & Wellness Manager (EHWM) and the Associate Director of Behavioral Health & Supportive Services (ADBHSS) in achieving CARES strategic objectives related to HIV care completion.  This includes support of CARES’ Healthy University (HU) and Nutrition programs in particular, as well as, broader support in a context of services to all people living with HIV (PLWHIV) served by the agency. The Associate specifically addresses HIV related health & wellness management with clients and does so from experiential, knowledge, and skill related contexts. The Associate provides these services through interpersonal health education/support, HIV medical/social services systems navigation support, and on-site program administration and monitoring. Moreover, the Associate shares his/her personal experiences/perspectives with clients/staff to promote client empowerment positively impacting long-term health outcomes and helping to alleviate HIV related stigmas.  


 

Specific Duties:

PROGRAM:

  1. Provides support in planning, scheduling, and implementing HealthyU & Nutrition Program activities;
  2. Recruits PLWHA to participate in program activities and promotes program activities within the community;
  3. Provides program support for Wellness Wednesday, Choice Pantry, Food Pantry & HealthyU activities;
  4. Provides education, skills training, support and follow-up to program clients on issues related to: living with HIV, medical/social services, HIV treatment adherence, sexual risk reduction, and overall health and wellness through the implementation of IAMSTRONG;
  5. Introduces and orients clients to Nashville CARES and community medical and/or social services;
  6. Attends/participates in peer volunteer trainings and/or meetings;
  7. Participates in interdisciplinary support to other services departments, including client eligibility and psycho-social support activities, as requested with program manager’s approval;
  8. Provides administrative, meal preparation and clean-up support for Healthy Relationships weekly; 
  9. Provides administrative, meal preparation and clean-up support for Healthy Relationships 2.0 bi-monthly;
  10. Completes all record keeping in ClientTrack according to assigned tasks;
  11. Remains knowledgeable on current issues regarding HIV health and access including attending webinars, trainings and lectures;
  12. Maintains the organization and cleanliness of the Programs Kitchen and assists in maintenance of the CARES Café.

General Duties:

  1. Adheres to project, agency, department and team policies and procedures;
  2. Participates in project, agency, department and team meetings, events, and strategic initiatives;
  3. Performs other duties as assigned by supervisor or agency.

 

Qualifications:

  1. Bachelor degree preferred or equivalent experience desired;
  2. Living with HIV is required;
  3. Professional and/or volunteer experience working with HIV+ clients desired;
  4. Professional and/or volunteer experience working in a social service setting an asset;
  5. Ability to maintain flexible schedule; availability to work day, evening and weekend hours required;
  6. Experience with computers, word processing and databases an asset;
  7. Group facilitation experience an asset;
  8. Must have excellent communication and interpersonal skills;
  9. Must have knowledge of issues affecting people living with HIV/AIDS;
  10. Must be able to physically lift 30 pounds;
  11. Driver’s license, clean DMV record, and automobile liability insurance required.
