Mission: LGBT Life Center is a trusted leader that empowers the LGBTQ communities and all people affected by HIV through improving health and wellness, strengthening families and communities, and providing transformative education and advocacy.
Essential Job Functions:
- Conducts HIV and STI testing and education to individuals at risk for infection in community-based settings.
- Engages communities into services through community outreach (e.g., use of online platforms and social media and out of office field outreach when feasible).
- Facilitates individual (one-on-one) or group health sessions that address risk behavior, stigma, and skills building activities.
- Coordinates and navigates care for individuals with HIV/STI concerns.
- Assess needs, identify resources, address barriers to care and linkage, and provide transportation assistance.
Responsibilities – Program Implementation and Oversight:
- Supports the establishment of working relationships, collaborations, and partnerships with community organizations to increase the network of services available to persons impacted by HIV and STI throughout our service area.
- Maintains a small caseload, supporting and coaching them to achieve their goals and work towards wellness, and self-sufficiency.
- Maintains meaningful involvement with clients, community partners, and internal stakeholders to ensure full integration of services and effective referrals.
- Provides accurate, complete, and timely data and reports to department managers as needed.
- Records data for program outcomes and ensures client files are complete, accurate, and frequently updated to reflect all essential data including case notes, services plans and assessment, and are updated in accordance with agency guidelines and funding regulations.
- Provides health services that are culturally and population affirming to risk behaviors, and skills building opportunities.
- Coordinates projects, set priorities and timely manages implementation of these activities.
- Coordinates and advocates needed care, treatment, and services for clients.

Qualifications:
Education: Bachelor’s degree in health science, health education, or a related field is preferred. Certification to conduct HIV/STI screening including rapid testing or venipuncture is preferred. Proof of degree and training completion is required.
Job Knowledge and Skills: Knowledge and skills related to care coordination and navigation required. This position requires knowledge and sensitivity to the impact of HIV/AIDS and sensitivity to the needs of diverse audiences including those of various racial and ethnic backgrounds and life experiences (e.g., persons experiencing homelessness, mental illness, or substance use). Direct experience working with gay or same gender loving men, bisexual+, queer, genderqueer, trans or non-binary individuals is preferred.
Qualified applicants must have the ability to work in a clinical and community environment. Must possess the ability to identify and communicate effectively with the members of the intended population. Ability to work with multicultural and diverse populations required. Effective oral and written communication skills required. Excellent organizational, decision making, problem solving, and time management skills are required to handle multiple activities related to program implementation. High energy level and self-motivated. Computer knowledge should include Microsoft applications (Word, Excel, Outlook).
Physical Requirements:
Must have reliable transportation and acceptable driving record is required. Transporting of participants is required.
This work requires the following physical activities: walking, sitting, hearing/seeing, talking in person or on the phone. Occasional walking, bending, standing, and driving are needed. Work is performed in an office setting, and in home environments. Work is also performed in social and community settings, as necessary. Minimal to moderate lifting is required at times. Extended work schedule may apply to accommodate client’s needs.

Salary: Non-exempt, full-time position with pay range of $16.83/hour to $19.23/hour based on experience, PTO, comprehensive benefit package including 403B, medical and dental coverage.
Preferred location: There is opportunity for some remote/virtual work, however applicants must live within 40 miles of LGBT Life Center offices in the Eastern Region of VA.
