Job Description:
This position works with a designated clinic care team to provide comprehensive case management, home visits and support for identified high-need patients. This position also provides health promotion to patients and serves as a health promotion instructor for designated class curriculum. This position also provides periodic staffing support for outreach events.
Qualifications:
Bachelor’s degree (BA/BS) in social service, health education, or public health preferred. Equivalent training, continuing education, and or experience may be considered in lieu of degree. Experience working with community organizations as an advocate for high-need individuals preferred. Case management tracking and documentation experiences or the ability to learn required.
