Highlighted Duties and Responsibilities:

  • Prepares or assists with the preparation of a variety of regular and special reports to include the Department of Health (DOH) monthly report, grants reports, etc.
  •  Creates, tables, charts, and graphs, to display patient data and grant deliverables.
  •  Attends all appropriate departmental staff and clinical meetings, including Quality Management, Healthy Start, and Prenatal Services team meetings as needed.
  •  Administers the Parent Survey and submits findings within 48 hours of completion to determine eligibility for program services and/or referrals in clients’ home or clinic environment.
  •  Ensures that prenatal patients are screened for Healthy Start prenatally or within three months of delivery.
  • Compiles required data in the EMR and Healthy Start database (salesforce) to document assessment, eligibility and appropriateness for programs per policy and procedure.
  •  Makes referrals to case management, housing, behavioral health and other services as needed.

Minimum Qualifications:

  • Bachelor’s degree in social work or related field or equivalent experience required.
  • If applicant has a BSW/MSW degree, LSWA/LGSW or appropriate social work license obtained within 90 days of hire.
  • Familiarity and experience with quantitative and qualitative data collection and analysis methodologies required.
  • Demonstrated cultural competence in communicating with underserved, diverse populations. Experience working with families preferred.
  • Strong interpersonal, customer service, communication and organizational skills.
  • Demonstrated capacity to input data accurately required. Experience working with EHRs or database system required.
  • Strong organizational skills required.
  • Ability to work flexible hours, including weekends and evenings.
  • Ability to conduct home visits. Valid driver’s license and vehicle required, as well as proof of auto insurance required.
