Job Summary

The Health Coach utilizes proactive, population health care activities to improve the health outcomes of WWH patients.

Core WWH Primary Essential Duties:

• Performs and behaves in accordance with Whitman-Walker’s mission, cultural norms, and core values of dignity, respect, affirmation, and humility.

• Maintains a respectful, non-judgmental, and compassionate manner with patients/clients/staff.

• Demonstrates excellent customer service by identifying and exceeding customer requirements.

• Adheres to Whitman-Walker policies and procedures, with special attention given to HIPAA requirements.

• Maintains data integrity through conscientious use of relevant tools and employing a system of checks and balances.

• Demonstrates organizational skills necessary to multi-task, meet deadlines, and re-prioritize as needed.

• Participates in organizational quality and performance improvement activities.

Role Specific Primary Essential Duties:

• Provide in-person and virtual training and technical support to patients on home-based or technologically-mediated monitoring (i.e. blood pressure, blood glucose).

• Executes interventions based on quality measure reports or other data tools (e.g., care gap reports).

• Facilitate health education projects; chronic disease self-management, goal setting, follow up, and promote/encourage behavior change.

• Assist patients and clinical team in completing preventive health screenings and health maintenance activities.

• Deliver patient education on topics including but not limited to chronic health conditions, diagnostic screenings, wellness activities.

• Connects patients to their primary care provider and/or specialist.

Professional

• Assists in health promotion activities related to care coordination programs such as MyHealthGPS

• Cross-trained in other areas of medical support in order to support as needs arise (hospitalization tracking and patient education and training)

• Clearly document clinical information and education provided in the electronic medical record

• Support special/pilot projects designed to innovate around care delivery and improve patient outcomes

• Other duties as assigned.
