’ (Lantern) mission is to champion the independence and well-being of New Yorkers who are impacted by or threatened with homelessness.  The lantern provides innovative services to help New Yorkers who are homeless, formerly homeless or have recently aged out of foster care recreate their lives. Our programs with proven results in health, employment, education, and life skills are delivered inside our residences and tailored to each person’s needs.  We provide our clients - many of whom live with mental illness, addiction, disability, HIV or other chronic illnesses with the tools they need to integrate successfully into the greater community with dignity and independence. 

Objective:

We are seeking a creative and engaging Health and Wellness Coordinator to design and implement programs in our Supportive Housing buildings aimed at increasing behaviors that lead to the optimal health and wellness of our clients.  The Health and Wellness Coordinator will be expected to conduct significant outreach to our clients and collaborate with internal and external partners to promote health and wellness initiatives.  The ideal candidate will have experience with health promotion, education, and community partnerships.  Additionally, the Health and Wellness Coordinator should have experience in working with people impacted by mental illness, substance abuse, criminal justice, and medical issues and using evidence-based models.

Location: Sites in Bronx, NY

Position Type:

This is a full-time position with a regular daytime schedule that may include 1-2 evening/late shifts.  Lantern staff may be asked to change the work schedules and hours depending on the operational needs of the site and the agency.   

Essential Functions:

  • Outreach and engage the client population in health and wellness programming, as well as other Special Programs.
  • Become knowledgeable of building a culture related to wellness, and best practices for client engagement.
  • Conduct outreach within the building to assess and engage clients.
  • Work with Case Management to identify high-risk clients and develop strategies to engage them in risk-related Lantern services and programming.
  • Develop and implement programming to ensure all clients have access to information and services based on best practices in the delivery of health and wellness programming. Implement evidence-based and promising practice models (such as Motivational Interviewing and Illness Management and Recovery) with fidelity.
  • Build partnerships and linkages with progressive and/or alternative community agencies that serve the needs of clients. Utilize these partnerships to coordinate programs and events in the building which provide opportunities for relevant community organizations to share their information and services directly with Lantern clients. 
  • Maintain an updated community resources directory.
  • Design and maintain health and wellness bulletin boards in each building.
  • Identify and schedule ongoing workshops with community providers who can share subject matter expertise in educating the clients.  Vet the providers to ensure that educational workshops are culturally competent for Lantern’s population.
  • Promote and facilitate a harm reduction or other health and wellness drop-in group in each building, as well as other health-related programs.
  • Train staff and serve as a program expert in the provision of best practices and progressive services and programming related to health and wellness.
  • Participate in weekly clinical case conferences, regular staff meetings, individual supervision and other meetings as required.
  • Document all client work in confidential electronic records.
  • Complete reports as directed by the supervisor.
  • Perform other job-related functions, as assigned.
