Waikiki Health is now recruiting for a full time EIS Medical Case Manager. The EIS Medical Case Manager will work with members of the interdisciplinary team to provide quality care to patients living with HIV/AIDS and affected others to achieve optimal clinical outcomes. The EIS Medical Case Manager will provide medication adherence counseling, health education and care management; coordinates the implementation of individual care plans and monitors referrals. The EIS Medical Case Manager reports directly to the HIV/EIS Manager.

Duties include but are not limited to:

  • Engage in a multi-disciplinary team approach in the development and management of care plans and appropriate medical and psychological interventions and referrals for people living with HIV/AIDS
  • Regularly assesses patient adherence to HIV medication and provides medication adherence/resistance education and self-management tools as needed
  • Reviews clinical records and performs coding activities monthly
  • Empower patients with education on basic health information, health care coverage options, and access to other health and community resources

Qualifications:

  • Flexible attitude and ability to work in a team environment
  • Flexible hours, including some weekend and evening availability
  • Good computer skills, with ability to work with a variety of program required
  • Valid Hawaii driver’s license
  • Bachelor’s degree in Social Work, Public Health, or other relevant concentrations from an accredited university preferred; relevant social work and/or health care experience may be substituted
  • Case management experience and knowledge of basic medical terminology is preferred. Experience working with individuals with HIV/AIDS is preferred
