The HIV Prevention Navigator is responsible for tasks related to managing STD clinic client cases for those seeking services at the STD clinic and who may benefit from HIV prevention services. This individual will provide brief intervention, PrEP/PEP education, linkage to PrEP/PEP, benefits navigation, linkage to services, and PrEP/PEP follow-up.

Principal Duties & Responsibilities:

  • Work with the HIV Prevention Counselors and HIV Prevention Coordinator to meet all Public Health Solutions (PHS) contract deliverables.
  • Collaborate with the HIV Prevention Counselors, STD clinic staff, and program staff at The Center’s main site to coordinate care and services.
  • Provide brief interventions and linkage to care, including PrEP/PEP providers, benefits providers, and other providers as needed.
  • Implement H-PLUS screening and complete client assessments.
  • Enter contacts and documentation into PHS and Center databases.
  • Other duties as assigned.

Position Requirements:

  • BSW or similar required; Associate’s degree with related work experience accepted; LMSW/LMHC a plus.
  • Previous clinical experience working in the field of HIV prevention and/or treatment. Prior knowledge of PrEP/PEP preferred.
  • Significant experience working with LGBT populations, including knowledge of the spectrum of gender identity and transgender issues.
  • Good administrative, organization, and written/verbal communication skills; bilingual a plus.
  • Commitment to inclusive, multi-cultural programming; prior experience working within coalitions with diverse communities and an understanding of intersectional oppressions.
  • A strong commitment to social justice and the mission of The Lesbian, Gay, Bisexual and Transgender Community Center.
