POSITION SUMMARY:

Under the direction of the Clinic Nurse Manager, provide overall management of the Home Health program. Oversee program compliance with the Home Health Joint AIDS Case Management Protocols as established by the California State Office of AIDS and Standards of Care established be the Los Angeles Commission on HIV/AIDS Health Services.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provide overall management of Home Health staff, budget and contract management and program development, implementation and evaluation.
  • Assist staff in addressing difficult or complex client issues by consulting on resources and providing education, technical assistance and problem solving.
  • Facilitate the enrollment of clients onto the County HBCM and Medi-Cal Waiver programs, and the provision of services to those clients as outlined in the Joint AIDS Case Management Protocols, and the MediCal Inpatient/Outpatient Provider Manual.
  • Oversee the establishment of care plans and the ongoing case management of the enrolled clients.
  • Monitor and oversee the performance of subcontractors, including the emotional support providers and the nursing/attendant care providers.
  • Ensure program quality through the implementation and execution of a quality assurance program to include client record reviews as outlined in the State Protocols.
  • Timely submit all service and data reports including semi-annual State progress reports, State QM plan and other documentation as required by funding sources and APLA Administration.
  • Perform program outreach and maintain relationships with community and internal referral sources.
  • Participate in community meetings and functions as a representative of the Home Health Program.
  • Develop departmental budgets on a yearly basis in accordance with Finance policies and procedures. Manage departmental operations to meet financial goals.
  • Hire, train, and supervise personnel assigned to the Home Health Program.
  • Respond to inquiries and grievances from clients, volunteers, and community members.
  • Maintain the quality assurance program.
  • Prepare statistical, program reports for planning and contract monitoring purposes, and submit them to the Clinic Nurse Manager in a timely manner.
