JOB SUMMARY 

The Housing Case Manager will provide comprehensive housing case management services to Positive Impact Health Centers (PIHC) clients who receive housing assistance via the agency’s HOPWA programs. The Housing Case Management role includes: developing a housing plan to address barriers to stable housing; connecting the client with medical care to address HIV; and coordinating supportive services that help the client reach identified goals.

ESSENTIAL FUNCTIONS

Provide housing case management services to PIHC’s TBRA and self-managed clients seeking housing assistance.

  • Meet with PIHC HOPWA clients in person to assist clients in developing stable housing plans and achieving identified goals.   Monitor PIHC HOPWA clients’ adherence to and engagement in medical care and report medical concerns appropriately.
  • Connect PIHC HOPWA clients with necessary resources including, but not limited to, housing resources, mental health services, medical care, food stamps, educational programs, employment resources and substance abuse treatment. 
  • Assist PIHC clients at all project sites with PIHC housing information and applications as needed.
  • Field a high volume of housing-related inquiries.
  • Document all housing-related tasks in designated electronic systems (HMIS, EMR).
  • Monitor HOPWA project goals and work to achieve HOPWA contract deliverables.
  • Develop collaborative partnerships with private and corporate landlords for TBRA and low-income housing candidates.
  • Communicate HUD housing procedures to landlords.
  • Attend HOPWA, HUD, housing, CPR/First Aid, professional development, HIV/AIDS and case management training annually/when available.
  • Represent the agency at HOPWA and housing-related community activities as needed. 
  • Maintain and update all housing related spreadsheets.
  • Assist with completing APRs and project monitoring annually, or as needed.

Required Knowledge, Skills and Personal Qualifications:

1. Experience working with People Living With HIV/AIDS (PLWHA) and culturally diverse populations

2. Possess excellent communication skills (writing, spelling, listening and speaking)

3. Ability to develop tangible Hosing Plans for homeless clients.

4. Ability to comply with Fair Housing Laws and Housing First Model.

5. Ability to translate federal housing regulations into plan text for client and landlord comprehension.

6. Ability to follows through to completion of assigned tasks

7. Proficiency in word processing and spreadsheet database 

8. Abides by all policies and procedures concerning personnel including conflict of interest policy

9. Experience with Microsoft Excel, Word and PowerPoint

10. Must possess excellent interpersonal skills and ability to communicate effectively in a multi-cultural setting.

12. Willing to travel between PIHC centers and to housing facilities as needed (25% of total work time).

Education and Experience

Bachelor’s Degree in Social Work or related field 

And

Minimum of three years’ experience in housing, social work, or case management.
