POSITION SUMMARY:

Under the Direction of the Program Manager of Housing Support Services, promote the health, well-being, and housing stability of people living with HIV/AIDS through the provision of life skills training including but not limited to, personal finance counseling, basic budgeting, employment counseling and continuing education.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Develop curricula for Life Skills training workshops.
  • Facilitate Life Skills training workshops.
  • Work one/one with clients who require more individual life skills counseling.
  • Identify specialists and agencies that can provide additional technical assistance to clients and facilitate workshops for clients receiving HOWPA services.
  • Assist Housing Specialists with incorporating life skills and health promoting objectives in the individual housing plan.
  • Work with the Program Manager to assess the efficacy of the life skills intervention.
  • Maintain and update all client records and progress note all interactions via the FamCare database program as needed.
  • Examine the training needs of staff in the area of Life Skills and develop and facilitate training as needed.
  • Maintain a resource directory of resources clients may access to promote income stability.
  • Monitor the APLA website to ensure that housing and treatment related information on APLA’s website is up-to-date.
  • Actively participate on Community Advisory Boards (CABs) of research institutes conducting housing research.
  • Assist in linking clients to APLA services/programs and other community agencies/programs.
  • Monitor referrals and follow-up on those requiring additional advocacy.
  • Prepare charts and forms in accordance with HOPWA and APLA policies and procedures.
  • Attend mandatory HOPWA training meetings.
  • Attend staff meetings and individual supervision meetings at APLA.
  • Participate in client conferencing with Housing Support Services staff when indicated.
  • Assure client confidentiality as defined by APLA policies and procedures.
  • Contact appropriate professionals to assist in the determination of need and/or eligibility for services.
  • Prepare documents and/or reports as it relates to the requirements of various funding sources.
  • Attend regularly scheduled in-services, as well as outside conferences, workshops, etc. to become updated on the latest referrals and information necessary to respond to client needs.
  • Education and inform APLA staff about changes in the housing continuum.
