Description: Cascade AIDS Project (CAP) is the oldest and largest AIDS Service Organization (ASO) in Oregon and Southwest Washington. We are seeking a part-time (.6 FTE)Minority AIDS Initiative (MAI) African American Services Navigator to provide strengths-based social services to people living with HIV/AIDS (PLWHA) who are African American, and to assist clients in navigating medical care, mental health care, and drug/alcohol systems, and provide support to clients in connecting to housing case management services, as this is critical to their stability and medical engagement. Also, facilitate a monthly support group, called Real Talk, for African Americans living with HIV/AIDS.

Responsibilities include: assessing client needs, providing information and referral, developing individual goal plans and assisting clients in carrying out the activities to achieve their goals, completing forms and entering data into the agency database in a timely and accurate manner, assessing clients’ adherence to HIV medications on a regular basis, supporting clients in building health literacy skills, working collaboratively with housing case managers and community partners, and other duties as assigned.

This position is based in our Portland office, but will travel throughout the five-county service area for meetings and client home-visits.  Some evening and weekend work is required.  This is a non-management, union-represented position.
