Founded in 1983 and one of the first AIDS organizations established in the United States, the AIDS Support Group of Cape Cod (ASGCC) works to foster health, independence and dignity for people living with HIV/AIDS and Viral Hepatitis by providing care, support and housing. ASGCC works to help reduce the spread of HIV and other sexually transmitted infections through prevention, education and testing services. ASGCC is at the forefront of the fight against the opioid epidemic which disproportionately affects the Cape Cod region.

The AIDS Support Group of Cape Cod is seeking a motivated and energetic person to fill the position of Medical Case Manager.  This is a full-time position working primarily traditional hours, Monday through Friday; the position is based out of Provincetown, with periodic office hours in Hyannis and other Cape-wide agency sites.  Responsibilities include conducting intake, assessments and screenings for new and registered clients, developing individualized treatment plans and serving as an advocate for clients in helping obtain a full range of benefits, including food and nutrition services and housing assistance.  This position will work in collaboration with Client Services and Prevention/Screening staff as well as other health providers and social services agencies to ensure that client needs are met as effectively as possible.    

Preferred candidates will have a basic knowledge of case management methods – including local and state resources available to people living with HIV/AIDS – and will possess a strong knowledge of HIV/AIDS issues, with a keen ability to maintain confidentiality.  Candidates should possess an interest in advocating for disenfranchised populations, and the challenges of working within a system with limited resources for same.  Also, successful candidates will have very strong verbal and written communication skills, a strong sense of professionalism and boundaries, an ability to problem-solve, and a sound ability to conduct crisis intervention and conflict resolution.  Must have a valid driver’s license.  Successful candidates will have 2-years of experience in human services including working with LGBT clients, people with chronic illness, those with substance use and mental health issues, the disabled and the homeless population.  Knowledge of Harm Reduction/Risk Reduction models of service delivery strongly preferred.
