The AIDS Foundation of Chicago (AFC) mobilizes communities to create equity and justice for people living with and vulnerable to HIV and related chronic diseases. We envision a world in which people living with HIV and related chronic diseases will thrive, and new HIV infections will be rare. Our work focuses on improving health equity, preventing new cases of HIV, serving as a collaboration and knowledge center, and being a bold voice for change.

In order to achieve our goals we are seeking an individual to serve as one of our CommunityLinks Mobile Engagement Team Members. The CommunityLinks Program serves clients in Managed Care Organizations (MCO) as part of AFC’s efforts to provide high quality services to vulnerable populations as health reform and the Affordable Care Act are implemented. The Team Member is an integral part of the CommunityLinks Mobile Engagement Team.

The Mobile Engagement Team Member conducts activities to locate MCO members, connect them to their health plan and offer additional services as required.  The position employs a number of strategies to engage highly vulnerable members who may be initially distrustful of health care providers or have been “lost to care.” The position will assess managed care plan members and offer HIV and hepatitis C (HCV) testing services when appropriate.  The position will assess managed care plan members and administer health risk assessments as needed.  They are also responsible for completing and submitting all forms and documentation.

Principal functions and responsibilities of this position include the following:

  • Locate assigned managed care members via various strategies working with internal Mobile Engagement Team (MET);
  • Administer and complete health risk assessments with located Members within timeframe, as needed;
  • Answer questions about the member’s health care plan and provide contact information for the health care plan;
  • Offer and administer HCV/HIV testing via oral or finger stick test and completes all required documentation;
  • Collect and process specimen samples in accordance with National, State, and Local guidelines;
  • Conduct pre- and post-test counseling as needed and required;
  • Elicit member’s HIV risk history including, but not limited to, demographic information, sex history, drug and alcohol use, and past STD infections;
  • Provide initial Linkage to Care counseling and services and complete required documentation for members who test positive;
  • Document CommunityLinks encounters, attempts, and services daily; prepares reports as needed;
  • Maintain the highest level of security and confidentiality at all times for public health information (PHI) by following all agency protocols;
  • Stay abreast of current issues related to specialty care, such as HIV or hepatitis C, through trainings, professional publications and research;
  • Attend in-service training and meetings as required by the CommunityLinks team and AIDS Foundation of Chicago;
  • Assist with agency-wide activities as directed including Annual Meeting, AIDS Run & Walk, World of Chocolate and others;
  • Protect organization’s value and manage risk by keeping information confidential;
  • Perform other duties as assigned.

Qualifications:

Minimum Qualifications

Associate’s degree (i.e., Business Administration, Liberal Arts, Human Services) OR At least six months related experience and/or training.

Ability to obtain Counseling Testing Referral Partner Notification (CTRPN) training within six months of employment.

Basic proficiency with Microsoft Office.

Preferred Qualifications

Bachelor’s degree (i.e., Public Health, Liberal Arts, Human Services) OR At least 2 years related experience and/or training.

Valid Illinois driver’s license, proof of liability insurance, and access to reliable transportation.

Fluent in Spanish.

Proficiency with Microsoft Office Excel or Databases.

Experience and/or training in motivational interviewing, harm reduction and trauma-informed care.

Knowledge of or experience working in the health care or health insurance environment.

Certified in using testing diagnostics, Clearview, OraQuick, OraSure and Biolythical for HCV and/or HIV.

 

Knowledge, Skills, and Abilities

Requires Strong attention to detail and interpersonal skills. The ability to provide efficient, quality service to both internal and external customers.  This includes putting the customer first, following through on responsibilities, ensuring that promised work is completed, taking responsibility for commitments.

Knowledge of the causes of homelessness, who it affects, and the factors (e.g., racial inequities, Socio Economic Status) that can contribute to an individual being homeless.  This includes the ability to understand the culture of homelessness and engage homeless or formerly homeless populations.

The ability to engage with clients in conversation and build trust and rapport to determine their needs.  This includes the ability to establish clear and appropriate boundaries and to discuss issues/violations when they occur. Solid verbal/written communication skills.

Also the ability to prioritize, plan, and coordinate work activities, and manage resources so that work objectives are accomplished efficiently are key to success in this role.
