The AIDS Foundation of Chicago (AFC) is seeking a motivated and reliable person to serve as a CommunityLinks Mobile Engagement Team (MET) Program Specialist. The CommunityLinks Program serves clients in Managed Care Organizations (MCO) as part of AFC’s efforts to provide high quality services to vulnerable populations as health reform and the Affordable Care Act are implemented. The Program Specialist is an integral part of the CommunityLinks Mobile Engagement Team.

The MET Program Specialist supports the activities of the Mobile Engagement Team. The MET conducts activities to locate MCO members, connect them to their health plan and offer additional services as required. The team employs a number of strategies to engage highly vulnerable members who may be initially distrustful of health care providers or have been “lost to care.” The MET Program Specialist will provide administrative support to the team, including scheduling and reporting, as well as conducting member searches and administering health risk assessments both in an office-based setting and out in the community. The MET Program Specialist is also responsible for completing and submitting all forms and documentation.

Principal functions and responsibilities of this position include the following:

  • Coordinate programmatic services for assigned monthly Member caseload;
  • Maintain proper documentation  of all daily programmatic activities: CommunityLinks encounters, searches, contact attempts, and field logs; prepare reports as needed;
  • Administer and complete health risk assessments with located Members within timeframe, as needed;
  • Plan and implement monthly outreach actives/services; 
  • Assess Member needs and connect to appropriate MCO supportive services;
  • Perform regular monitoring of data quality in Client Track;
  • Facilitate weekly MET planning meetings, set MET field schedule and ensure MET members receive all necessary information for member location activities;
  • Respond to internal requests for information regarding services provided;
  • Protect organization’s value and manage risk by keeping information confidential;
  • Maintain the highest level of security and confidentiality at all times for public health information (PHI) by following all agency protocols;
  • Represent CommunityLinks at meetings with MCO representatives;
  • Prepare and submit service reports required by MCOs and AFC’s finance department for invoice purposes;
  • Locate assigned managed care members via various strategies working with internal Mobile Engagement Team (MET);
  • Offer and administer HCV/HIV testing via oral or finger stick test and completes all required documentation in accordance with National, State, and Local guidelines;
  • Attend in-service training and meetings as required by the CommunityLinks team and AIDS Foundation of Chicago;
  • Assist with agency-wide activities as directed, including Annual Meeting, AIDS Run & Walk, and others;
  • Protect organization’s value and manage risk by keeping information confidential;
  • Perform other duties as assigned.
