SUMMARY

The Patient Navigator is responsible for providing education to assist HIV patients with staying in care. Maintaining contact with eligible patients and providing support to patients including assessing barriers, needs and acuity, and developing, implementing and monitoring an action plan for the patient. This role works closely with the Fulton County Health Department and other community providers and serves as a communication link with groups of social service agencies that provide service to persons with HIV.

 

QUALIFICATIONS

  • Bachelors degree in health, social services or a related field required
    • High School/GED accepted with five (5) years of direct HIV/ AIDS experience
  • Two (2) or more years experience in Infectious Disease preferred
