Description:

The Long Island Association for AIDS Care, Inc. is looking for a Peer Navigator to assist with engaging HIV+ consumers.  The Peer Navigator will accompany consumers on appointments, provide encouragement as well as coaching and assist with monitoring of progress. This is a part-time position (10 hours/week) that pays $15.00 per hour.

 

Responsibilities:

  • Conduct outreach and engagement efforts to engage and retain consumers in care. 
  • Coach consumers in self-advocacy and self-management. 
  • Accompany consumers on clinical and supportive service appointments. 
  • Enroll consumers who are eligible into ADAP and/or other public private health care coverage programs. 
  • Assist Case Managers during some interventions. 
  • Assist the Health Educator during educational sessions.

 

Requirements: 

Openly living with HIV/AIDs and able to share personal experiences.  Must be familiar with navigating the systems of HIV care and be a consumer of services. Also must be knowledgeable about the region’s services, the communities and populations being served and have excellent written and verbal communication skills. Must be familiar with print publications and social media sites that cater to the HIV+ population.
