The William F. Ryan Community Health Network is a group of not-for-profit, federally qualified health centers that delivers high quality, affordable and comprehensive medical care to diverse and underserved communities. We are currently seeking a PrEP Coordinator for our William F. Ryan Community Health Center located on the Upper West Side of Manhattan.

 

Position Overview:

The PrEP Network Coordinator works in conjunction with the Ryan Network’s HIV Services leadership staff to manage, promote, and expand the delivery of PrEP and PEP services within Ryan Network. This encompasses supervision and training of staff (clinical and non-clinical), outreach and HIV testing activities, patient education and navigation, and community education. This work is carried out in support of the mission and goals of Ryan Network.

Essential Functions:  

Management, Supervision and Quality Assurance/Quality Improvement

  • Assist with the administration of the Ryan Network’s PrEP and PEP programs, including: staff recruiting, hiring, training, and evaluating; developing program policies, procedures, and workflows; and completing regulatory and fiscal reports to ensure compliance with funding requirements, program deliverables, and regulatory agency mandates.
  • Ensure the timely completion of all PrEP-related data entry and reports via the use of eCW, AIRS, eSHARE, and other electronic databases.
  • Implement quality improvement and performance improvement initiatives related to performance and clinical outcomes associated with the Ryan Network PrEP and PEP program.
  • Develop and implement PrEP and PEP marketing strategies with Ryan Network’s Social Media Coordinator, External Affairs staff, and offsite vendors.

Patient Engagement and Navigation

  • Conduct promotion and patient recruitment for Ryan Network’s PrEP and PEP program, with emphasis in venues/locations/neighborhoods where target population(s) (e.g., high-risk negative individuals, young men who have sex with men, women, etc.) convene.
  • Provide benefit and entitlement navigation for patients to receive PrEP and PEP services within Ryan Network.
  • Complete non-medical patient assessments for PrEP services.
  • Conduct care planning activities (e.g., scheduling appointments, monitoring appointment adherence, preparing bi-annual care plans, identifying service gaps, case closure, etc.) for PrEP patients in partnership with their larger care team (e.g., medical provider, nursing staff, etc.).
  • Conduct rapid HIV testing utilizing all available methods in clinical and non-clinical settings.

 Other

  • Develop linkages between health care and other services providers (e.g., mental health, substance use, supportive services, etc.) to assure continuity of care for patients.
  • Represent the Ryan Network in community fora, collaboratives, and other settings as appropriate.
  • Practice infection control measures in accordance with OSHA’s Bloodborne Pathogens Standards.
  • Attend required meetings, case conferences, and supervision as assigned.
  • Immediately report problems or unusual occurrences to immediate supervisor, or other Ryan Network leadership as appropriate.
  • Perform other duties as assigned.

 

Minimum Experience and Skills Required:

  • Minimum 2 years of experience providing HIV educational or social services to the target population(s).
  • Knowledge of HIV prevention and treatment
  • Strong computer literacy with proficiency in standard office software/hardware such as Microsoft Word, Excel, Outlook, PowerPoint.
  • Effective communication and writing skills.
  • Excellent problem solver capability with a demonstrated history of process improvement.
  • Proven track record of organization, attention to detail and ability to respond promptly to requests, anticipate organizational needs, and maintain efficient office operations.

 

Education, Licenses and/or Certifications Required:

Bachelor’s Degree in Health or Human Services required or a related field.

 

Preferred Qualifications:              

  • Master’s Degree in Public Health or related field.
  • Minimum 1 year of supervisory experience.
  • Bilingual (English/Spanish).
