Venice Family Clinic is a community health center whose mission is “To provide quality primary health care to people in need.” Venice Family Clinic cares for over 28,000 low-income patients in over 120,000 visits annually who might otherwise go without critically needed health care services.

Responsibilities

Under the supervision of the Prevention Supervisor, the Pre-Exposure Prophylaxis (PrEP) Navigator provides intensive care coordination, support and services to HIV negative individuals who require assistance in accessing or remaining in PrEP care. You will provide patient support and navigation services to HIV negative individuals who are at higher risk of HIV and/or who may have barriers to accessing and adhering to PrEP care. These services will be provided either in -person or via phone.

You will work with HIV testing and clinical staff to identify potential PrEP clients and those in need of PrEP navigation services. You will provide support to HIV negative individuals who are interested in starting PrEP and those already utilizing PrEP care. You will assess benefits and insurance, and if needed, provide assistance to obtaining those.  You will provide HIV prevention and PrEP adherence education and counseling. You will make referrals for external services needed to support PrEP care and follow up to ensure service delivery. You will provide ongoing follow up to ensure PrEP adherence. You will serve as a fully functioning member of our team, able to participate in daily activities and functions to maintain overall Common Ground operations.    

Qualifications

  • Robust knowledge of PrEP/PEP
  • Knowledge of HIV/AIDS, community resources, health education methods, advocacy, harm reduction and general clinical knowledge
  • Ability to complete required program documentation
  • Public speaking and leadership ability
  • Skill in providing individualized HIV health education
  • Ability to discuss sensitive matters in a non-judgmental manner and to respect confidentiality
  • Ability to work effectively with people of diverse cultural and educational backgrounds
  • Skill in maintaining cooperative working relationships with staff members and volunteers
  • Skill in creating flyers and other program-promoting documents
  • Bilingual in conversational Spanish.
