PRIMARY FUNCTIONS
 
Under general supervision, provides direct covered services and treatment to individual clients, coordinates with community support and stakeholders.  Responsible for managing appointment schedule, assigned caseload, conducting assessments, performing crisis services, facilitating transition and participating in ongoing learning experiences and treatment team consultation and coordination.
 
SPECIALIZATIONS

Registered Nurse or knowledge and abilities equivalent to a Registered Nurse.
 

 
MINIMUM QUALIFICATIONS
 
  • Will have a bachelor’s degree or associates degree from an accredited college in nursing and licensed as a registered nurse (RN) or equivalent knowledge and abilities with a minimum of two years of full time behavioral health work experience or demonstrate professional qualifications.
  • Knowledge of DSM5 diagnosis
  • Able to work a flexible schedule as needed per program requirements.
  • Must successfully complete the agency’s orientation and demonstrate competence regarding skills and knowledge per R9-20-204.
  • Have good organization and time management skills.
  • Valid Driver’s License
 
JOB RESPONSIBILITIES AND COMPETENCIES  
 
  • Experience in psychosocial assessment of clients
  • Experience in interdisciplinary care coordination; monitoring of health and social service delivery to maximize efficient/cost effectiveness.
  • Capable of providing reports on weekly and monthly basis.
  • Possess good written and verbal communication skills.
  • To communicate and coordinate services with other treatment providers when appropriate.
  • Attend and actively participate in team staffing.
  • Attend primary medical care appointments with consumers when needed.
  • Provide medication adherence and medication monitoring.
  • Ability to effectively engage client’s and in some cases, their family members in treatment
  • Provide non-judgmental, unconditional support to clients.
  • Experience in working independently
  • Must be capable of working with diverse cultural population.
  • Knowledge of community resources and ability to assist clients in securing such services.
  • Must have basic computer skills.
  • Must have access to safe transportation, valid Arizona driver’s license and automobile insurance as required in the State of Arizona.  Must be willing to travel throughout Maricopa County.
  • Discharge functions completed within required time periods.
  • Maintains and monitors caseload statistical information, including length of stay and outcome measures when assigned.
  • To provide on call coverage for crisis situations as needed.
  • Understanding of all licensing, regulatory and agency guidelines.
  • Document significant information in each file in a clear and timely manner.
  • Consults and coordinates with prescriber, prescriber liaison, laboratories, medical providers, pharmacies, and assists with eligibility and access to prescription services when needed.
  • Consults on initial and ongoing psychiatric evaluation requests.
  • Monitor appropriate productivity needs, variables, and opportunities
  • Coordinates and conducts group functions and facilitates cohesiveness in its entirety that demonstrates improvement in a client’s/family’s symptoms and functioning level.
  • Conducts community-based services, as needed
  • Demonstrates ability to de-escalate and de-brief disruptions to group and /or family content and process.
  • Ability to assist clients to learn skills required to successfully engender psychological stability and personal independence.
  • Recognize symptoms to the full and broad range of DSM diagnoses.
  • Ability to provide routine, urgent, and emergent interventions.
  • Application and integration of American Society of Addiction Medicine criteria in service planning and implementation.
  • Ability to apply motivational enhancement, stages of change, family systems, and appropriate family communication process.
  • Ability to understand and screen for serious mental illness, and complete requests for determination.
  • The ability to obtain and maintain fingerprint clearance card, First Aid, and CPR.
