As someone who helps coordinate care in the western region of Montana (and who has worked in HIV since 1986 in New York City until 2002) I was annoyed by sweeping statements based on 2 clients self reports. The most obvious misstatement to me if regarding the first client. Anyone on Sustiva and Combivir must stop the Sustiva first and remain on Combivir alone until the Sustiva clears the system. This is best practice and may be what was happening with that client. In any area you can find a couple of people who receive less than optimal care for a number of reasons but I can attest to or rigorous pursuit of the guidelines and frequent calls to the warmline in San Francisco to check unusual situations. When you are in "paradise" again we'd love to have you visit our clinic.

Paul, thank you for sharing your experiences as you travel. I too love Montana but recognize the expertise in HIV care remains uneven -- depending on who your physician might be. Thanks again for your blog and updates. I'm reading every word. Megan

I have been to a Gay mens retreat at Featherpipe Ranch...what a blast that was. Did you get to walk around the camp......and go to the memorial site? That's awesome....we have lost many friends to this disease....and they are all honored there !! Good reading.....take care and have a great day !!
