Anal Pap is as simple as a cervical or vaginal pap. It isn't painful in the least. A number of cells lining the rectal wall are removed with a small brush and sent to pathology. I've been around awhile, & I've never heard of an anal pap smear. My ID doctor has never mentioned this. It sounds painful.
