No blind study is done, nor can it be ethically be done, to study if people who are knowingly HIV+ get more illnesse than people who are unknowingly HIV+. The HIV+ diagnosed person is given a HIV+ diagnosis and usually then put on anti HIV meds. So, we don't then know how much impact the anti HIV meds have on health, nor how much the fear / anxiety of knowing you are HIV+ plays out on your health. It is just assumed that it's the HIV making the person have more health issues.
