The article doesn't state whether or not the letter writer is poz or not. If he is, then he may be suffering with lypodystrophy. If that's the case, he can exercise until the cows come home. The question does not provide sufficient information about the health of the person and his poz status.

I have been poz for over 20 years. I deal with weight gain and it is hard to lose. I have lost 20 pounds and I have gained it back, I have lost 10 and I have gained it back, I seem to maintain a specific weight which leaves my BMI around 28. I strive to eat correctly and healthy and I watch my blood report on my cholesterol and any other thing that affects my life. I suggest focusing on healthy eating, on what is best for you, some form of activity every day. Be happy regardless.
