Most people react very negatively to someone who is doing something that is essentially suicide. Without antivirals, the average lifespan is a couple of years. Those years will become a living hell very quickly, with constant medical issues. We avoid all of that by taking our 'pill' daily and essentially live a normal life. One day, a cure will come along and the living will be cured. Those who did it 'their way' won't have the opportunity to be cured. That's only for the living.

Perhaps the friends putting this pressure on them to go on to ARVs do so subconsciously so that they them selves can be a bit irresponsible without contracting HIV them selves... Just a thought. Anyway it doesn't seem so stigma based but rather fear based and care based to me. They fear for them selves getting HIV. And their care for their friends who are HIV+ but not on ARVs.
