The currently on-going huge clinical trial that will determine when is the best time to start treatment is the START (Strategic Timing of AntiRetroviral Therapy) Study, conducted by the INSIGHT network. The SMART study, mentioned in your article, closed in January 2006. It was another mile-stone trial by the INSIGHT network. Its primary purpose was to determine whether treatment interruptions would be beneficial (e.g. by diminishing the long-term side-effects of the treatment and increasing adherence). The study found that treatment interruptions were associated with increased risks of HIV disease progression or death, serious HIV disease progression, and other severe complications.
