Responsibilities: Complete the adult comprehensive assessment and determine with the client (and co-workers when appropriate) the individual treatment and aftercare plans Responsible for client aftercare referrals (follow-up substance abuse treatment) and placement Promote ongoing participation of the client with decisions involving plans, goals and progress Adult comprehensive assessment, admission MIS form, and treatment plan to be completed within first 24-48 hours after admission Arrange aftercare interviews or other related appointments by telephone after obtaining appropriate releases to disclose necessary information Coordinate treatment services with external treatment providers such as DCF workers, DMH workers, and outpatient counselors or therapists Coordinate discharge date with nurse and insurance coordinator Complete discharge summary and discharge MIS form Maintain relevant and timely counseling progress notes Provide a level of counseling conducive to a stable medical and psychological state during detox/withdrawal. Arrange referral to the BEST Team when indicated Lead educational/recovery client groups as assigned Attend daily report with other unit disciplines and communicate clients’ plans and progress as coordinated with team members Assist in team effort in handling crisis oriented emergencies Document all interventions in the counseling progress notes Available for weekend and holiday coverage in fair and equitable distribution Attend staff meetings, supervision, in-service conferences, periodic reviews and other relative meetings as determined by the clinical or program director Participate as an active member of the multidisciplinary treatment team Record data for tracking and reporting services to the Ryan White Foundation for Title1 services Serve as an advocate and resource, providing the client with community support information, assist clients to arrange essential appointments and accompany clients to those appointments requiring attention during their stay Responsible for conducting post-discharge follow-up for clients within 30 days of discharge Other case management duties as directed by the clinical director or program director

Qualifications: BA/BS or minimum four year experience in the human services field. Certification or eligibility for certification in substance abuse counseling or two years experience in substance abuse field and a willingness to pursue certification. Thorough knowledge and understanding of the federal regulations pertaining to confidentiality of substance abuse treatment.
