Position Summary: Responsible for screening, assessing, counseling and linking HIV-positive clients to needed services (e.g., housing, entitlements, medical/mental health care, treatment, food & clothing) both within and outside the agency.   

 

Essential Duties and Responsibilities:

  • Conduct intake interviews and needs assessments on newly released inmates and clients;
  • Create individualized treatment plans for each client;
  • Conduct crisis intervention with clients, as needed;
  • Assist clients in obtaining all necessary documentation to facilitate benefits application process;
  • Complete and submit clients’ application to appropriate benefits provider agency (e.g., HRA, Social Security);
  • Act as a liaison to representatives of benefits administrations;
  • Refer clients to appropriate service providers and follow-up on referral outcomes;
  • Refer clients to appropriate medical and/or mental health care providers;
  • Establish and maintain contacts with NYC based detox and treatment facilities, parole offices, shelters, medical providers and other community based organizations;
  • Escort individuals to referral appointments, especially those that are medical and housing related;
  • Advocate for client services, especially housing and HASA benefits;
  • Follow-up with all referral agencies regarding client progress;
  • Conduct client home visits;
  • Document all client services, including successful and unsuccessful outreach attempts, in client file;
  • Enter all client services in the agency’s database, as well as in the appropriate contract funder databases (AIRS)
  • Develop curriculum for and facilitate client support groups;
  • Conduct community outreach activities geared toward client recruitment and promotion of our agency’s services;
  • Conduct outreach within correctional facilities, as needed;
  • Prepare and submit reports on client intake and discharge activities;
  • Perform other related duties as assigned.

Qualifications:

  • High school diploma or GED/HSE required, Bachelor’s degree preferred;
  • At least 2 years of experience working on HIV/AIDS, Substance Abuse and/or Criminal Justice issues;
  • Experience working with clients, at-risk youth and other disenfranchised populations;
  • Experience with Microsoft Office and database entry;
  • Ability to encourage disenfranchised populations to see and obtain needed social services;
  • Strong verbal and written communications skills;
  • Bi-lingual English/Spanish a plus;
  • Strong follow-up, organizational and leadership skills;

Travel Requirements:  Requires occasional travel to our Long Island City location.
