Great opportunity for an experienced Case Manager. This position focuses on the support & clinical services of the general CHWC client. Assists with obtaining primary care and connects the client to other core services such as dental, substance abuse, and mental health services. Facilitate linkages and referrals to maintain the client’s involvement with their primary medical services. Ensures that the clients get assistance with any assistance applications such as medical, financial, housing, etc.

Required Qualifications:

  • Bachelor’s Degree in Social Work or a Human Services related field preferred. * Knowledge of case management systems and ability to work sensitively with a variety of cultures and lifestyles required. * Ability to function autonomously as a worker and as a team member on a multidisciplinary team. * Excellent oral and written communication skills, problem solving, and conflict resolution skills. * Familiarity with local/regional/state and federal health and human service resources.
