Highlighted Duties and Responsibilities:

  • Performs intake assessment and additional screening as needed.
  • Meets regularly with patients on case load to develop case plans and appropriate interventions. Monitors client progress through the course of treatment, re-evaluating and adapting the case plan at required intervals.
  • Attends weekly supervision with Behavioral Health Integration Manager.
  • Maintains appropriate scheduling and files in eCW, including progress notes, telephone encounters, virtual visit notes, and any document needing to be uploaded via patient docs within eCW.
  • Assists the Behavioral Health Specialist and coordinates with primary medical and dental care, including referring patients to community resources.
  • Accompanies patients to appointments when necessary and makes home visits when appropriate.
  • Provides case management support to the school based mental health counseling program, Medication Assistance Treatment Program and Behavioral Health program which includes but not limited to identifying and addressing psychosocial concerns, engagement in services, coordinating care, advocacy, and health related barriers.
  • Reviews and Monitors CRISP Reports for clients who have had a psychiatric hospitalization. Follows up with these clients to ensure they are connected to behavioral health services at Community of Hope or with a community mental health provider within seven days and a second follow-up within 30 days.
  • Coordinates with all health services, and other internal and external service providers regarding clinical care, service delivery and barriers to care. Consults with supervisor on difficult to engage clients.

 Minimum Qualifications:

  • Master’s degree in social work or related field required.
  • Minimum of two years work experience in health case management or social work.
  • Strong interpersonal skills.
  • Strong organizational skills.
  • Knowledge of community resources or the ability to become knowledgeable.
  • Experience working with underserved population preferred.
  • Demonstrated ability to multi-task and problem solve.
  • Ability to travel between sites required.
  • Ability to work flexible hours, including evenings and weekends, required.
