Broward House, Inc. is a not for profit  social service agency serving Broward County with multiple programs for individual infected and affected by HIV, homelessness, substance abuse and mental health issues

We are looking for an Intervention Case Manager to work in our Program working with MSM (men who have sex with men) at risk for or impacted by substance abuse, mental health and HIV.  The candidate will have a strong comfort level working with the target population as well as providing, individual and group prevention and intervention and case services addressing mental health, medical care, social supports, and other life areas.  Hours will vary, including weekend, evenings, and some late nights with a strong outreach component.

  • Provide education to targeted population to engage and recruit clients for mental health service and HIV and Substance abuse risk reduction
  •  Conduct effective outreach to target population
  • Enroll and complete assessment of individuals demonstrating risk factors
  • Develops an individual risk reduction plan based on identified client’s assessment, needs and strengths.
  • Executes intervention strategies with clients in individual or group strategies as indicated in risk reduction plan
  • Arranges for services and completes referrals required for client based on individualized needs.
  • Meets with internal and external interdisciplinary treatment team members to coordinate client services and documents outcomes of meetings
  • Arranges for discharge or transfer from services as dictated by program and client needs.  Provides follow-up and documents status in chart and electronic record as required.
  • Submits all documentation in an accurate and timely manner, including electronic records.  Achieves and maintains program established productivity levels.

Skills / Requirements

  • Minimally a Bachelor’s degree in a social service or health related field 2 years case management/social services experience, Masters Preferred
  • HIV/AIDS knowledge preferred
  • Knowledge of Substance Abuse and Mental Health Symptoms and interventions
  • Effective rapport building with individuals in target population
  • Solution focused approach to conflict and challenges
  • Ability to establish and maintain effective working relationships with employees, collaborating agencies and the public
