The mission of AIDS Services of Austin is to enhance the health and well-being of the community and people affected by HIV and AIDS. For more than 27 years, ASA has provided a continuum of services to the greater Central Texas area.

As a Medical Case Manager you will work in partnership with our LVN Medical Case Manager to support clients enrolled in our Positive Living thorough Understanding and Support (PLUS) program.  This short-term intensive program works with clients who are newly diagnosed or have barriers to HIV medication adherence. With compassion you will guide clients through a variety of case management activities focused on their medical care plan and individual goals. Your highly valuable medical experience is an incredible educational asset that will help your clients feel knowledgeable and empowered in managing their HIV and co-morbid symptoms.

 

Ideal Candidate Profile:

 

You are a master communicator: This role works hand in hand with the LVN Medical Case Manager; the two case managers share most clients and case consult at least weekly. You must be able to work independently and communicate your progress effectively to the team.

 

You’re a mover and a shaker: Effortlessly, you move from appointments, to calls, to notes, to meetings, and back again. You thrive on a full schedule with lots of variety, which will include home visits, office appointments, and accompanying clients on doctor and hospital visits.

 

You’re ’Type A’ organized: You’re on top of your notes, and have extra time at the end of the day to review your client files!

 

You’re patient: Problem solving and mentoring come easily to you.  You enjoy being of service.  When you help someone to a personal breakthrough it makes all the struggles worthwhile!

A successful Medical Case Manager must be willing and able to:

  • Provide clear information to clients and case managers.
  • Advocate on behalf of the client to their clinicians and your interdisciplinary team.
  • Help clients develop a care plan that focuses on immediate and long term goals.
  • Maintain accurate and up to date client data in our electronic medical records database.
  • Adhere to the guidelines of our grants’ documentation requirements.
  • Function within the Agency’s policies and Procedures as outlined in our Personnel Handbook.
  • Achieve positive client outcomes.
  • Demonstrate computer literacy.

 You Have:

  • A bachelor’s degree in a social services field
  • A LBSW license (preferred)
  • Two years in case management or medical social work, preferably with experience in HIV, mental health, substance use disorders, or with other medically vulnerable populations

Super Extra Credit for:

  • A master’s degree in a social services field
  • A Texas LMSW licensure
  • Bilingual/bicultural (English and Spanish)
  • Two years of experience working with individuals of diverse cultures, ethnicities, socioeconomic backgrounds, sexual orientations, and gender identities and/or expressions
  • Training in harm reduction principles and motivational interviewing
