About LIAAC:

The Long Island Association for AIDS Care, Inc. (LIAAC) is a community-based, not-for-profit agency that provides services and support for Long Islanders infected and affected by HIV/AIDS and at high risk for HIV infection and other infectious diseases. Our staff utilizes a plethora of community resources to refer and link clients to medical treatment, supportive services, and assist them with navigating through a sometimes daunting and complicated social service and health care system.

Basic Function: 

Responsible for overseeing and conducting Mobile Outreach activities via a field-based model that combines outreach, HIV/HCV/STI testing and Evidence Based interventions (Think Twice, Motivational Interviewing) with recovery support services (case management and linkage to care), and social marketing campaigns. The population of focus: male and female racial/ethnic minority individuals living with or at risk for HIV/AIDS ages 18 and older, with a special focus on racial/ethnic minority YMSM (ages 18-29), MSM (ages 30 and older), and gay, bisexual, transgender individuals, returning veterans, and population of focus’ needle sharing and/or sexual partners. 

Specific Responsibilities: 

  1. Supervise 2 prevention specialists; conduct documented bi-weekly supervisions and monthly team meetings with staff, review all staffs’ documentation and data entries.
  2. Process requests for Mobile Outreach program promotion and testing events and complete staff assignments for various community and outreach events.
  3. Ensure workplan goals are being met; create Continuous Quality Improvement projects based on programmatic challenges.
  4. Set monthly deliverables for staff to ensure program objectives are being met.
  5. Facilitate Project Safety Net Consumer Advisory Board (CAB).
  6. Oversee the acquisition of literature and ensure culture, language, and literacy levels are appropriate.
  7. Represent the agency on committees in Nassau and Suffolk Counties.
  8. Prepare monthly, quarterly, semi-annual and annual SAMHSA reports.
  9. Design, implement, standardize, and streamline reporting mechanisms to facilitate accurate data collection, reporting, and compilation of related data.
  10. Maintain quality assurance systems to ensure that standards of care are met, services are in place for clients, documentation is up-to-date, and problems and crises are appropriately addressed and documented. Follow a Continuous Improvement model.
  11. Collaborate with the Director of Mobile Outreach and Deputy Chief Program Officer on staffing issues, staff development and long term planning.
  12. Represent the Director of Mobile Outreach at meetings within the agency and in the community, as assigned.
  13. Meet programmatic deliverables: targeted outreach, HIV/STI/HCV testing, Linkage to Care services, comprehensive case management services, condom distribution and education, conduct Think Twice intervention, and complete GPRA baseline and follow-up surveys with discharge paperwork.
  14. Other duties as assigned.

Qualifications:

BA with 3+ years’ of supervisory experience overseeing outreach activities targeting high-risk communities, knowledge of community resources and SUD/COD required. Bilingual preferred.
