Under the administrative direction of the Director of Access Services and the medical direction of a contracted Medical Physician, the RN Medical Case Manager assists HIV positive individuals and their significant others with issues as they relate to HIV/AIDS disease. This medical case manager will focus on client retention in medical care; adherence to medical appointments, medications and other medical service plan goals; and client development of the knowledge and self-sufficiency skills for independence in adhering to service treatment plans. This position will provide medical case management services as a member of the Access Services Department

Education, Training & Experience

  • Graduation from an accredited school of nursing required
  • Current Texas licensure as a Licensed Registered Nurse required
  • One year of experience with HIV/AIDS disease preferred
  • Experience working with individuals of diverse cultures, ethnicities, sexual orientations, and gender identifications preferred
  • Bilingual in English & Spanish

Essential Tasks

  • Consistent with the Austin TGA and agency case management standards, provide day-to-day nursing medical case management services for ASA clients.
  • Develop a comprehensive nursing assessment that includes client HIV and other diagnosed medical history, client primary medical problems, review of HIV medications and lab values
  • Complete physical examinations including vital signs and observation of client physical health
  • Develop medical service plans with clients and their medical team that focus on goals outlined in the medical treatment plan, such as adherence to medications, scheduled medical appointments, and specialty care follow-up, and detailing nursing interventions; and monitor the plan to reassess for effectiveness
  • Coordinate services/medical treatments, provide required advocacy and communicate with the clinical team in order to implement the medical plan;
  • Complete nursing interventions such as medical counseling on medication adherence, side-effects, and lab results; referral to clinical trials or specialty care; addressing health literacy issues; and actions related to serving as medical liaison between primary medical care team, pharmacy, other providers
  • Conduct ongoing routine client follow-up on medical treatment and service plan goals through home/hospital visits and/or telephone contacts to reassess needs
  • Assist clients and their significant others with complex issues related to HIV and AIDS disease in accessing and retaining primary medical care through advocacy, resource linkage, and supportive counseling.
  • Maintain documentation and program notes in the client records according to departmental standards utilizing a computerized client database.
  • Participate in program planning and policy formulation for Access Services Department.
  • Participate in quality improvement activities.
  • Complete additional duties as directed.
